Transitional Housing

St. Leonard’s HPE offers long-term transitional housing for youth between the ages of 15 and 24 who need safe, stable housing, life-skills development and the sense of permanence needed to reach their full potential in their chosen community.

Program Details

We offer a four room, congregate transitional housing space in Quinte West, Ontario. The space is supported by 24/7 staff, a dedicated team member to oversee shared responsibilities in addition to one-to-one case management and vital transportation support.

Program Goals

The goal of St. Leonard’s HPE Supported Independent living Program is preparing youth for sustainable independence. By providing youth the opportunity to develop life-skills, attend school and/or training programs, build community connections and restore natural systems of support, our housing program will ensure those who leave are ready, and prepared to contribute to their communities.

Youth between the ages of 15 and 24, who are without safe, alternative living arrangements, and who are engaged in education, employment, trades or other training programs, may be eligible to apply.

Youth must complete a application, demonstrate interest in life-skills development, and be currently enrolled in school or employed.

Following an approved application, our management team will conduct a placement interview with interested youth and their supports as applicable. An approved application does not indicate space. Youth eligible will be contacted by our leadership team following their placement interview.

Any agency or organization supporting eligible youth may make an inquiry with the youth seeking services present. Self-referrals are welcome.

Our team must speak directly to the individual seeking services in order to complete a referral/assessment for eligibility and to email/send an application to be filled out.

We are not accepting applications from external agencies at this time.
